Nominations may only be submitted by the Director of Athletics. Each form must be submitted electronically by May 17, 2024 for consideration.
After submitting the form, please email the following documents to cdswoy@gmail.com, with the header “Nomination Letters.”
- Director of Athletics
- Head Coach (one letter from each coach if multiple sport nomination)
